Medication Therapy Management at University Health helps patients with multiple chronic conditions manage their medications. We aim to increase your comfort and safety by reducing the possibility for drug-related complications.
What Medication Therapy Management Can Do for You
If you qualify, we will reach out to you to discuss your medication plan. We’ll answer your questions and provide information that can help you better understand your medications. Often, we provide these services by phone. You can also schedule an in-person appointment.
Services
Eligible patients receive services for free.
Medication Therapy Management includes a one-on-one meeting with a pharmacist to:
- Find cheaper medications
- Make sure you are using your medication correctly
- Reduce medication-related problems
- Find new medications that can improve your health
- Help you make positive lifestyle changes
- Learn more about your condition
A pharmacist will call you to schedule an appointment. Each session lasts about 30 minutes. You will receive a personal medication record and a medication action plan with advice on how to solve medication problems.
Eligibility
You must meet certain criteria to be eligible for this program. You must have Medicare Part B and:
- Have at least one of these chronic conditions – diabetes, high cholesterol, high blood pressure, asthma or chronic pain
- Take 10 or more prescription medications
- Spend at least $4,000 per year on prescription medications
We get a list of all eligible patients and call them to let them know they qualify.
