Leaving the Hospital
Assisting You and Your Family
Discharge from a hospital stay is often a time of relief. It’s also a time to prepare for what’s ahead. For some, rehabilitation or follow-up appointments will need to be scheduled. You may also have special at-home care instructions or need referrals.
Your care team will assist you and your family with discharge planning.
Pick Up Valuables
Once you’re aware of the date of your discharge, you can return to the Admissions Office to pick up your valuables. You will need:
- Your copy of Form 95
- Photo ID
Arrange to pick them up during these hours:
- Monday through Friday, 6 a.m. to 7 p.m.
- Saturday, 7 a.m. to 3 p.m.
- Sunday, 7 a.m. to 1 p.m.
Outside of these hours, you can pick up your valuables by going to the Emergency Department, on the Ground Level of the Sky Tower, using the A Elevator. The front desk clerk will contact the Patient Care Coordinator to retrieve your valuables.
Going Home Checklist
Be sure to make a list of the things you and your family need to do before you leave the hospital:
- Arrange transportation home
- Pack your personal belongings
- Retrieve any valuables from the hospital safe in the Admissions Office
- Take any medications, vitamins or herbs you brought from home
- Leave a forwarding address for mail
- Make financial arrangements regarding payment of your bill
- Check with your nurse concerning any special instructions for your care at home
- Confirm date, time and location of follow-up appointments
Medical Supplies & Discharge Instructions
It’s important to follow any discharge instructions from your nurse or physician for continued quality health care at home. If you have specific discharge requests or needs, please tell your nurse. We will help make any arrangements as needed and provide you with:
- Necessary medical supplies and equipment
- Verbal and written discharge instructions concerning appointments, medications and prescriptions
Filling Prescriptions at Our Pharmacies
Prescriptions for discharge medications may be filled at the hospital's Outpatient Pharmacy or a pharmacy of your choice.
- Our Outpatient Pharmacy is located in University Hospital, first floor, in the Rio Tower, past the B Elevators.
- University Health pharmacies are conveniently located around San Antonio.
Patients who are treated in our Emergency Department can get prescriptions filled at our After-Hours Pharmacy located on the ground floor, in the Rio Tower, D Elevators.
