Preparing for Your Pediatric Gastroenterology Appointment

Being well-prepared for your child’s digestive health appointment at University Health can lead to more accurate diagnoses and better treatment options.

What to Bring to Your First GI Appointment

Ensure a helpful visit by bringing the following:

  • Referral forms: If your insurance requires a referral, bring the completed forms from your child’s pediatrician or family doctor.
  • Pharmacy contact information: Have your pharmacy name and phone number available.
  • Lab & radiology reports: Previous results from any lab or imaging tests.
  • Growth curves: If available, a copy of your child’s recent growth curves.
  • Guardian documentation: A parent or legal guardian must be present at appointments. If applicable, please bring proof of guardianship or legal documentation.
  • Medications list: A detailed list of your child's medications, including prescriptions, over-the-counter drugs and herbal remedies. Please include the medication name, dosage, frequency and the prescribing provider’s name, or bring all medicines in a plastic bag.
