At University Health, patients, families and caregivers are essential partners in care. This belief is why we have our Patient and Family Resource Center available to all patients and their support systems.

Through this resource center, we are committed to fostering meaningful and purposeful collaboration among patients, families, caregivers, team members and administrators to ensure that every voice is heard and every experience matters.

Patient-and Family-Centered Care at University Health

Our mission is to advance the delivery of high-quality, compassionate care by actively engaging those we serve. Through the core concepts of Patient- and Family-Centered Care (PFCC), we work together to create a health care environment rooted in dignity, respect, communication and partnership.

What is Patient-and Family-Centered Care (PFCC)?

Collaboration

Patients, families, health care practitioners and health care leaders collaborate in policy and program development, as well as implementation and evaluation, whether that be in facility design, professional education, research and in the delivery of care.

Information Sharing

Health care practitioners communicate and share complete and unbiased information with patients and families in ways that are affirming and helpful. Patients and their families receive timely, complete and accurate information to effectively participate in their care and decision-making.

Participation

Patients and families are encouraged and supported by participating in the care and decision-making at the level they choose.

Respect and Dignity

Health care practitioners listen to and honor patient and family perspectives and choices. Patient and family knowledge, values, beliefs and cultural backgrounds are incorporated into the planning and delivery of care.

How We Bring PFCC to Life

  • Active Patient and Family Advisory Councils (PFACs) supporting programs, policies, training and research
  • PFCC Training for all Patient Family Advisors, University Health team members (clinical, administrative and support), and community and academic partners
  • Collaborative committees that include patients, families, caregivers, providers, team members and administrators, along with community and academic partners
  • Transparent communication and shared decision-making across the continuum of care
